Abstract-Alternative: Hospital role in district health care activities may be involved in comprehensive medical service for the whole life of the district people, including clinical care, preventive medicine and public health nursing. Presented here is a review focused on public health nursing service of our hospital. The activities were initiated in 1922 with children's health consultant business because of high mortality of infants and milk stations for poor nutrition after the great earthquake, resulting in a leading maternity hospital in 1923. School clinic was first demonstrated in Japan in 1924 and well baby clinic in 1925. Public health nurse (P.H.N.) department was officially nominated in 1925. The service was extended to home visit for T.B. patients, class of baby-and-patient food and infant clothes and prophylactic education of venereal disease. In 1930 newly-started medical social service joined P.H.N. activities with good help. P.H.N. group also worked up model services in rural districts in 1933-4 with good reputation. The initial public health center of Tokyo started with some physicians and 23 P.H.N. who moved in from our hospital, planting their works in 1935. The remained St. Luke's P.H.N. service was welcomed by middle and hight class people. During the world war II they worked very hard for home visits, health management of factories, kindergartens, emergency care for the wounded, etc. After the war home visits for postoperative patients greatly increased in number because of shortness of beds resulting from the occupation of our hospital for the allied forces' army hospital. After the hospital was released from 11-year occupation P.H.N. service has functioned with well baby clinic, well mother clinic, mother school, maternity school, chronic disease clinic, health management of hospital staff and home visits. Services for chronic patients are gaining weight. In the stream of changes of social situation and medical needs for varied disease structures and patterns P.H.N. services have been adapted to function well for the purposes intended. P.H.N. services may get more energy with elastic selection of service targets and voluntary actions in good hospital activities. Hereafter a closer communication with other public health centers may promote possible further development of district health care activities.
